• Assist with Plan Review meetings and help you understand the NDIS process and to understand your plan
• Provide impartial advice to give you choice and control over the providers you use
• Assist with transitioning:

Out of hospital
Out of home care
• Guide you through your NDIS plan as you choose services to match your needs
• Connect you with service providers, community resources and local activities
• Resolve issues with any of the services provided under your NDIS plan
• Ensure agreements are in place with providers to guarantee the successful delivery of services
• Assist with housing and Individualised living options
• Complete referrals for services and assist with initial bookings
• Support to monitor your NDIS funding and achieve your goals
• Assist with Psychosocial Recovery Coaching
• Support to connect to complex care providers
